I used the "NORMAL" Niacinamid 500mg and NO problem or so for a 30+ year old very healthy slim adult. When i did reorder they shipped the sustained release and without knowledge of that i took 1 pill and after 2 days i had extrem pain in the stomach region and dark pee for the next 14 days. At times i thought i have to go to emergency. After finding out the studies of "sustain-release" induced niacin causing hepatitis and liver toxicity i was kind of released that the cause of my terrible condition was external and therefore in future easy to control, at the same time disappointed that this huge negative side effect is not red printed at IHERB to avoid suffering. If you are like me dealing with lyme disease and hyperactivity of NMDA receptors in the brain, then Niacin works fantastic. Niacin, as a precursor of NADH, helps to block the creation of quinolinic acid by a negative feedback loop in the kynurenine pathway. Quinolinic acid is a potent NMDA receptor stimulator and causes toxicity and hyperactivity in the brain. Lyme is potent inducer of quinolinic acid, and so Niacin helps to combat that. The internet says Niacin deficiency may present as ulcer formation, dizziness, bad breath, headaches, nausea, irritability, nerves, depression and stress. Presumably if you are low then supplementation will restore levels and ease these conditions. I was hoping it would fix bad breath, low blood pressure, and my dermatitis as a bonus as they mentioned but it was a no for me lol. Must have another cause. But I'm not stressed about it, which is on the list, ha.WARNINGS: High doses should be avoided in: diabetes, low blood pressure, gout, liver disease or ulcers. I have two conditions on the list, low blood pressure and the gout that is firmly under control by eating very low carb and low oxalates, and I feel ok to take this Niacin. My Natropath says one pill is a low dose and B3 is water soluble so any excess goes down the toilet.

My family has a history of cholesterol problems even when eating healthy and working out! I was even at 7% body fat, had minimal saturated body fat in my diet, and working out 6 days a week and still was in the high range. I work in the healthcare industry and learned about niacin and the effects it has on the bad cholesterol and good cholesterol. I decided to give this a spin and have been on it for several weeks. I tapered my dose to minimize the flushing effects from niacin (i.e I split the pill and only took 250 mg initially). I can say I've had minimal events where I had flushed skin and I believe it is because I took this approach. THIS PRODUCT IS NOT TO BE USED FOR LIPID CONTROL.When I had a near-lethal reaction to a popular prescription statin (lipid-lowering) drug, I decided to switch to niacin to control my elevated lipids. "Lipids" include cholesterol, triglycerides, HDL (the "good" cholesterol) and LDL (the "bad" cholesterol). No one, not my primary care doctor nor my cardiologist, informed me that there are (at least) three kinds of niacin on the market. Naturally, I picked the wrong one first (inositol), which the cardiologist later told me does nothing to control lipids. My primary care physician had no idea why my liver enzymes were suddenly elevated, and didn't make the connection to this version of niacin, nor did she realize that this product was the likely culprit for the fact that I was throwing up a lot.Only after doing my own research and finally asking the right questions in a search engine did I discover that "sustained release" and "extended release" niacin are NOT the same thing. The former (this product) can hurt you very badly if you're taking it at the maximum therapeutic niacin dose (4 pills a day). The latter is not known to cause elevation of liver enzymes, and thus is a good choice for lipid control.Again, "sustained release" niacin is also known as niacinamide and nicotinamide. If the label has any of those names on it, DO NOT take it if lipid control is what you're looking for.Extended release niacin is also known as nicotinic acid. The prescription version brand name is Niaspan; the generic name is niacin ER. If the label says "extended release" and/or "nicotinic acid," this is the correct product for lipid control.I have also been informed that any product that ONLY says "niacin" on the label (and does NOT mention niacinamide or nicotinamide) is also nicotinic acid, but (as best I can tell) is released/absorbed quickly, which can cause hot flashes. I am also told that taking a 325 mg aspirin tablet 30 minutes before taking the niacin might help with the hot flashes.So, in summary, if you are taking niacin for lipid control, DO NOT TAKE THIS PRODUCT. Its label simply says "nutritional health." I don't know what that means, but apparently it has nothing to do with lowering cholesterol and triglycerides.
